As a safety leader, how do you get a better understanding of how safe a company is at a glance? Or maybe a better question, what metrics are used to help make meaningful decisions about safety – especially when it comes to preventing serious injury?

TRIR (Total Recordable Incident Rate) has been one of the most widely used safety metrics for a long time, but a new trend has emerged over the last couple of years. Because a lower recordable incident rate doesn’t automatically mean the most serious risks are under control, more safety leaders are looking beyond TRIR and putting greater focus on SIF (Serious Injury and Fatality) prevention – identifying and controlling the hazards most likely to cause life-changing or fatal harm.

In this blog, we’ll explain what TRIR measures, how SIF prevention works, and how safety teams can use both together to build a stronger, more proactive safety program.

What is TRIR?

TRIR – Total Recordable Incident Rate – is a way OSHA measures whether a company's workplace is actually getting safer. Think of it as a report card for safety performance: it tracks the number of recordable injuries and illnesses per 100 full-time workers over a given year, giving teams a consistent way to benchmark themselves across sites, industries, or time.

A "recordable" incident is anything beyond basic first aid – think days away from work, restricted duties, job transfers, loss of consciousness, or a significant diagnosed injury or illness.

The reason it's calculated as a rate rather than a raw number? A company with 2,000 employees will always log more incidents than one with 200, but that doesn't mean it's less safe. TRIR levels the playing field by accounting for total hours worked, so you're comparing frequency, not just volume.

In short: the lower your TRIR, the safer your workplace.

How is TRIR calculated?

Knowing what TRIR measures is one thing – but understanding how it's calculated helps you interpret what the number is actually telling you.

TRIR is calculated using the following formula:

TRIR = Number of OSHA-recordable incidents × 200,000 ÷ total hours worked

The 200,000 figure is a standardized constant. It represents 100 full-time employees each working 40 hours per week for 50 weeks – essentially a full year of work for 100 people. Using this constant is what allows companies of different sizes to compare their rates on an even playing field.

For example, if a company records 3 injuries in a year and its workforce logs a total of 100,000 hours, the TRIR would be 6.0. That means for every 100 full-time equivalent workers, 6 recordable injuries occurred over the course of the year.

What that number means in practice depends on the industry. TRIR benchmarks vary significantly - a rate considered acceptable in one sector may be a red flag in another.

According to the Bureau of Labor Statistics, the 2024 incidence rate of total recordable cases in private industry was 2.3 cases per 100 full-time equivalent workers, the lowest rate for this data series going back to 2003.

But that number is only a broad benchmark. Rates vary by industry and even more by subsector. Manufacturing averaged 2.7 in 2024, while construction overall was 2.2. However, specific construction subsectors can run higher. Safety teams should compare TRIR against industry-specific benchmarks rather than one universal number.

Why TRIR still matters

TRIR is still widely used for good reasons. It gives companies a standardized way to track recordable injury frequency, compare performance across sites and time periods, and demonstrate safety progress to leadership, customers, insurance providers, and business partners.

So, reducing TRIR matters. Fewer recordable injuries is always a good thing. But TRIR should be viewed as one part of the safety picture, not the whole picture. It can show whether recordable incidents are going down, but it doesn’t always show whether the most serious hazards are actually under control.

What TRIR doesn’t tell you

Here's where TRIR can create blind spots: it measures how often recordable incidents happen, not how severe those incidents are.

Every recordable injury counts the same way in the formula. A minor laceration requiring treatment and a life-altering event both move the rate by exactly the same amount. Meanwhile, a near miss with serious injury potential may not appear in the data at all. That means TRIR treats all recordables as equal when, from a risk perspective, they are anything but.

Research backs this up in a significant way. A peer-reviewed study by Hallowell, Quashne, Salas, Jones, MacLean, and Quinn - published in the Professional Safety journal and based on an analysis of 3.26 trillion work hours across 10 construction organizations - found no discernible statistical association between TRIR and fatalities. In other words, trends in recordable injury rates do not reliably predict whether serious injuries or fatalities are increasing or decreasing.

The same study found that TRIR fluctuates so much due to factors outside a safety program's control that comparing one company's rate to another's – or even tracking year-over-year changes – rarely tells you what you think it does. For most organizations, the difference between a 1.2 and a 1.6 isn't a meaningful signal.

The takeaway isn't that TRIR is useless – it's that a declining TRIR does not automatically prove that serious injury risk is going down with it. A company can have a low and improving TRIR while high-consequence hazards remain unaddressed.

This is exactly where SIF prevention becomes important.

What is SIF prevention?

In safety, SIF stands for Serious Injury and Fatality. SIF prevention focuses on identifying, controlling, and reducing hazards that have the potential to cause life-changing or fatal injuries.

The key difference between TRIR and SIF prevention is that SIF looks beyond the number of recordable incidents. It asks a more important question: Where could someone be seriously hurt, even if no one has been yet?

Most serious risks do not always show up in TRIR. Near misses, close calls, potential cut hazards, and struck-by exposures may not affect the recordable incident rate at all. But each one can reveal a serious gap in how risk is being controlled.

This is why SIF prevention is more focused on potential severity. Instead of treating all recordables the same, it looks at the tasks, conditions, and exposures most likely to lead to potential catastrophic harm.

In the field, that means looking closely at things like:

  • High-energy work
  • Sharp materials and cut hazards
  • Pinch points and impact hazards
  • Dropped objects
  • Struck-by exposures
  • Electrical and arc flash hazards
  • Work at heights
  • Heavy equipment interaction
  • Chemical splash or liquid exposure
  • Heat, flame, and burn risks

The overall goal is to find the hazards that could change someone’s life and make sure the right controls are in place before something happens.

We’ve covered how TRIR and SIF prevention each play a role in workplace safety, but it helps to see them side by side. The important thing is that this should not be viewed as TRIR or SIF; safety teams need both.

How safety teams can identify SIF potential

The next step is learning how to spot serious injury potential before an event becomes catastrophic.

This does not mean every near miss or minor injury needs to be treated the same way. The goal is to look at the conditions around the event and ask whether the outcome could have been much worse.

A simple way to evaluate SIF potential is to ask.

  • Was there a high-risk exposure?
  • Were the right controls missing, skipped, or not followed?
  • Was the task allowed to continue despite the risk?

For example, a worker may avoid injury after a dropped object lands nearby. On paper, that may not affect TRIR. But if the task involved overhead work, poor tool control, missing head protection, or workers inside the drop zone, the exposure still had serious injury potential.

The same thinking applies to cut hazards, pinch points, struck-by exposures, electrical work, chemical splash risks, or any task where one missed control could lead to life-changing harm.

This is where SIF prevention becomes more useful than simply reviewing incident counts. It helps safety teams learn from serious-risk situations before they turn into serious outcomes.

Overall, TRIR can help show whether recordable injuries are increasing or decreasing. SIF prevention helps show whether the most serious hazards are being identified and controlled before they lead to a severe outcome. Together, they give safety teams a more complete view of workplace risk.

Where PPE fits into serious injury risk reduction

PPE sits at the bottom of the Hierarchy of Controls; it's the last line of defense, not the first. Engineering controls, administrative changes, and elimination of hazards should always come first. But in the real world, PPE is almost always part of the equation, and when the hazard has serious injury potential, the stakes for getting it right are much higher.

For safety teams focused on SIF prevention, the question isn't just "does this worker have PPE?" It's more specific: if this task goes wrong, is the protection they're wearing actually matched to the worst credible outcome?

That means evaluating PPE by hazard, from head to toe - not just by job title or what's been standard on the site for years.

  • Head and face: Does the helmet stay secure during the actual work being performed? Does face protection provide the right coverage for the specific exposure - impact, chemical splash, or arc flash?
  • Eyes: Does eyewear stay on during the task, or are workers removing it because it fogs or fits poorly?
  • Hands and arms: Does the glove provide the right cut, puncture, or impact protection for the materials and tools being handled, or was it chosen because it was familiar?
  • Body: Does the garment meet the right arc flash or flame-resistant rating for the energy levels workers are actually exposed to?
  • Feet: Do safety boots provide the right combination of puncture resistance, toe protection, and traction for the surface and task at hand?

Each layer of protection matters, and a gap in any one of them can be the difference between a recordable incident and a life-altering injury.

This is where a PPE assessment can help – by evaluating real-world hazard exposures across the entire body and identifying where protection may not be matched to the risk. The goal isn’t PPE that checks a compliance box.

It’s protection that workers will actually wear, that performs when it matters most, and that’s selected with the worst credible outcome in mind.
